Pages from the Aging Parent Caregiver Binder: cover, welcome page and table of contents

One phone call, and suddenly you need everything.

"Mom fell." "Dad's in the ER." In that moment you need her medication list, his doctor's number, the insurance card, and the name of whoever has power of attorney. Usually that information lives in five places, or in one person's head.

This binder pulls it together so you can find what you need in an emergency, walk into every appointment prepared, and share the load with your family.

What's inside

A 56-page fillable PDF (US Letter) plus a 1-page Quick Start Guide with a first-7-days plan.

  • Health: medication tracker (3 pages), weekly med chart, appointment log, visit prep, symptom tracker (BP, blood sugar, weight, pain), hospital discharge checklist and stay log

Questions, answered

How do I fill it in?

Open the PDF in the free Adobe Acrobat Reader, type into the fields, and save. Some browser and preview apps may not save form entries reliably, so Acrobat Reader is recommended. You can also print it and write by hand.

Is this a Medicare guide?

No. It includes Medicare key dates, free help resources and a side-by-side worksheet so you can compare plans using numbers from Medicare's Plan Finder or a free SHIP counselor. Medicare information was checked against Medicare.gov and CMS in October 2026 and may change. It is not insurance advice.

When is Medicare Open Enrollment?

October 15 to December 7 each year. The comparison worksheet helps you line up premiums, deductibles, drug costs and doctor networks before you decide.

Does it include legal forms like a Power of Attorney?

No. The legal section is a checklist of common documents and a place to record where they are and who holds them. Requirements vary by state, so please work with a qualified attorney.

Can I share it with my siblings?

Print as many copies as your family needs for personal use. Please don't resell or redistribute the files.

How do I get it?

Right after checkout you get a download link for the 56-page binder and the 1-page Quick Start Guide. Nothing is shipped.
